Support should follow the individual’s needs, goals and choices, along with the services for which they are eligible and authorized.

Your Care Can Include Someone You Already Trust

Depending on the program and your eligibility, your caregiver may not have to be a stranger.

An eligible family member, friend, or other person you already know and trust may be able to become your paid caregiver through Solana Care Network.

We can help families understand the next steps and determine whether this may be an option for their situation.

2. We learn more about your situation.

We will talk with you about your needs, current services or program involvement, and whether you already have someone you would like to provide your care.

3. We help determine the next step.

Depending on your situation, that may include discussing available services, coordinating with your case manager or other professional, or beginning the process of bringing an eligible caregiver onto the Solana team.
